People can describe why they need a longer appointment, a quieter space or support with medicines.
The form includes eating and drinking, oral hygiene, sensory needs and support with medical procedures.
Clinical teams in London and Leeds reviewed the passport over several rounds. They focused on information they need during care: how a person communicates, what support helps and which adjustments to make.
The one-page summary gives staff a place to find those details before an appointment or hospital stay.
A flag tells staff a person needs adjustments. The code tells them which one. Know Me suggests the SNOMED CT code that matches each adjustment in the passport.
Participating NHS teams get a short coding summary with each shared passport. Every suggestion shows the person's own answer, so staff can check it fits before adding it to the record.

Know Me only suggests the codes. NHS teams review each one and update the Reasonable Adjustment Digital Flag in their own systems.
