Know Me for NHS organisations

Raise your reasonable adjustment digital flag compliance — with coded adjustments from a passport people fill in themselves

The digital flag (DAPB4019) is now mandatory. Know Me turns what a person records into record-ready SNOMED CT reasonable adjustments

Improve your digital flag compliance

The reasonable adjustment digital flag (DAPB4019) has been mandatory since December 2025, and full compliance has been required since 30 September 2026. We believe compliance means recording the adjustments as well as the disability diagnosis.

Phase 1 — record, flag & share locally

Record adjustments, flag the record and share them within your service. Know Me hands your teams the person's adjustments already structured and coded.

Phase 2 — integrate with the national flag

Upload and view adjustments through the flag on the National Care Records Service. Coded, category-complete data makes recording and national reporting easier.

Two ways in

People keep their health passport free. Organisations choose how much set-up and integration they want.

What your organisation gets

The person completes the passport in their own words. Know Me helps your teams use it.

Regional set-up

Set up for your area: the service team that receives passports, the wording people see when they send one, and who it is for. Leeds is configured this way today.

SNOMED CT coded adjustments

Know Me suggests the code from the NHS list that matches each adjustment. Staff check each suggestion against the person's own answer before adding it to the record.
