A health passport that becomes coded reasonable adjustments

We translate what a person records into Reasonable Adjustment SNOMED CT codes — ready for the patient record and the digital flag.

So your clinicians add meaningful adjustments in seconds — not a flag that only says "learning disability".

Working with the NHS

Shaped with NHS trusts, patients and clinicians

Developed with Leeds Teaching Hospitals NHS Trust — who supported focus groups with people with a learning disability and autistic people, and gathered feedback from clinicians across the trust. Accessible “Easy on the i” imagery by Leeds and York Partnership NHS Foundation Trust.

From a person's words to coded adjustments

Know Me does the coding your teams don't have time for — plain-English adjustments become record-ready SNOMED CT.

1. They complete a passport

The person, or their carer, records the adjustments they need in their own words — prompted across every category.

2. We translate to SNOMED CT

Know Me maps their answers to Reasonable Adjustment SNOMED CT codes and the flag's categories. Available to subscribing trusts.

3. Clinicians add them

Your clinician reviews the coded adjustments and adds them to the record and the digital flag — meaningful and shareable.

Every flag carries the standard's core code 1326341000000105 — Know Me adds the coded adjustments that make it useful.

One passport, translated into coded adjustments

A person fills in a Know Me health passport once. We translate it into the relevant Reasonable Adjustment SNOMED CT codes — every applicable one in the national standard (DAPB4019), across all 11 categories.

Reasonable Adjustment Digital Flag

One record, shared across every service

A flag that says what to do — not just a label

An impairment on its own tells a clinician a category. A meaningful flag tells them how to make care work for this person.

A flag on its own

"Learning disability"

Tells a clinician a label — but not what actually helps this person, so the same barriers repeat at every visit.

With Know Me

The adjustments that make care work

  • Extra time and plain, literal language
  • A quiet room with dimmed lighting
  • A familiar carer present for procedures
  • Numbing cream before blood tests

All coded to SNOMED CT and ready for the record.

Ready for the 30 September 2026 mandate

The digital flag standard (DAPB4019) has been mandatory since December 2025, with full compliance due by 30 September 2026.

Phase 1 — record, flag & share locally

Record adjustments, flag the record and share them within your service. Know Me hands your teams the person's adjustments already structured and coded.

Phase 2 — integrate with the national flag

Upload and view adjustments through the flag on the National Care Records Service. Coded, category-complete data makes recording — and national reporting — far easier.

Built for subscribing NHS trusts

People keep their health passport free. Your trust subscribes to the function that turns it into coded, record-ready reasonable adjustments.

SNOMED CT translation

Plain-English adjustments become Reasonable Adjustment SNOMED CT codes — no manual coding for your teams.

Record- and flag-ready

Coded adjustments are ready to add to the record and the digital flag on the National Care Records Service.

Category-complete

Prompts cover every category, so flags are meaningful and recording is complete for national reporting.

Low friction

The person does the capture. Clinicians spend seconds confirming coded adjustments, not minutes typing them.
